Restricted animals and plants may be obtained by completing a USDA PPQ Form 526 (Application and Permit to Move Live Plant Pests or Noxious Weeds). The permit-approval process may take up to 90 days, and the USDA strongly recommends electronic submission for fastest response (first option below). International customers: other permits may be required for restricted shipments; please contact us at 800.334.5551 (Mon-Fri, 7am to 8pm, ET) for more information.
After applying for and receiving your USDA PPQ Form 526 (instructions below), please fax it to us at 800.222.7112 or email it to customer_service@carolina.com. We will keep the permit on record until its expiration date to make future purchases easy.
The USDA offers two, convenient ways to complete and submit your application:
- Apply Online or by Email
The fastest way to complete the formand to receive your permitis online at the USDA website. After registering with an account on their website, you can fill out Form 526, and submit it electronically.

- Apply by Mail or Fax
You may also fill in the application by hand and mail or fax it to the USDA.
